My Care Companion

Imagine every patient having natural conversations with a digital Care Companion that provides the personalized care support they need, including:

• asking patients to prepare for an upcoming visit with a provider (pre-visit planning)

• prompting patients in-between visits to discuss the status of their care plan or any questions they may have

• discussing the care plan they just received from a care provider (post-visit follow-up)

• assisting patients at any time based on their personal needs and goals

The Care Companion engages patients shortly before each visit to capture an up-to-date “About Me” profile which gives providers a deeper understanding of each individual, allowing for treatment on a more personal level. The Care Companion is available to anyone using a cell phone or tablet SMS messaging app.
